Required Documents
- Professional licenses (RN/LPN) or certifications (CNA/TMA)
- Current CPR certification card
- COVID-19 vaccination documentation
- Valid driver's license
- Social Security card
- Immigration documentation (passport, green card, or work permit)
Health & Training
- TB testing within the past year (2-Step TB skin test, Chest X-ray within 5 years, or Quantiferon test within 5 years)
- Completion of infection control training
- Completion of abuse prevention training
- Completion of dementia care training
